Welcome to the registration form for Western University's math challenge program 2025-26. Math Challenge is Western University's problem-solving program for Grades 5 - 12, led by the department members and local math teachers. In this program students tackle challenging problems to build critical thinking and prepare for math concepts. The program runs for different grades on different levels from weekdays from October to April.

Deadline to register is January 22, 2026

The form consists of a few sections to collect various pieces of information, including:

• Personal information of the participant
• Emergency contact information
• Media (Photo/Video/Audio) Release and Consent form (must be signed and filled out by a parent/guardian)
• Participant Code of Conduct and Parent/Guardian Acknowledgment (must be signed and filled out by a parent/guardian)
• Registration fee payment (Payment methods accepted are Visa, MasterCard and Visa Debit).The registration fee is $130.00 CAD (+HST).
