Welcome to the registration form for Western University's math camp in the summer of 2025. The form consists of a number of sections to collect various pieces of information, including:

• Personal information of the participant
• Emergency contact information
• Photo release consent form (must be signed and filled out by a parent/guardian)
• Registration fee payment (Payment methods accepted are Visa, MasterCard and Visa Debit)

The registration fee of $250 Canadian dollars (plus taxes) will cover expenses for lunches and refreshments during the three days of the camp, gifts, prizes, supplies as well as administrative expense. Cancellation fees will be applied in case of cancellation.

• β-Camp (Beta Camp) July 21-25, 2025. This camp is designed for students who will complete grade 8 in the school year 2024-25
• γ-Camp (Gamma Camp) July 7-11, 2025. This camp is designed for students who will complete grade 9 in the school year 2024-25. 
• θ-Camp (Theta Camp) July 14-18, 2025. This camp is designed for students who will complete grade 10 in the school year 2024-25. 
• ζ-Camp (Zeta Camp) July 28-August 1, 2025. This camp is designed for students who complete grade 11 or 12 in the school year 2024-25. 

Which camp would you like to attend? Choose the camp based on the grade that you are enrolled in currently (in the 2024-25 academic year). If you have attended Western Camp before, please do not choose the same camp. If you would like to attend an upper-year camp, please make sure to contact Dr. Ghorbanpour (aghorba@uwo.ca) before you register for one of the camp.

Family members of librarians, staff, and faculty at Western are eligible for a 15% discount. To receive the discount code, please send an email from your Western email account to aghorba@uwo.ca, including the department you work in.
