Welcome to the registration form for Western University's math challenge program 2024-25. The form consists of a number of sections to collect various pieces of information, including:

• Personal information of the participant
• Emergency contact information
• Photo release consent form (must be signed and filled out by a parent/guardian)
• Registration fee payment (Payment methods accepted are Visa, MasterCard and Visa Debit)

The registration fee for the entire program is $100 (plus taxes) per student. The registration fee will be used to cover program expenses, including supplies, administrative costs, staff and teacher hiring, as well as supporting initiatives to promote mathematics in the community.

Even though we state grade levels, these are a guide and should not be considered a restriction. With one registration you can attend more than one program, however, you will be able to choose only one of the programs in the registration form. IMPORTANT INFORMATION

• We won't have any classes during Christmas break nor on exam week.
• If city buses are not running or the university has been closed due to snow, there will be no classes.
• There are no books required for the program, but we can recommend books appropriate to augment the students’ studies.
• It is expected that students will work at home on the problems handed out and be prepared to discuss them in class.
• Please arrive at class ten minutes before the class is due to start so that we can start on time.
• We reserve the right to request students to leave who behave inappropriately and/or distract others.
