Welcome to the registration form for Western University's math challenge program 2023-24. The form consists of a number of sections to collect various pieces of information, including:

• Personal information of the participant
• Emergency contact information
• Photo release consent form (must be signed and filled out by a parent/guardian)
• Registration fee payment (Payment methods accepted are Visa, MasterCard and Visa Debit)

The registration fee of $100 Canadian dollars (plus taxes) will cover expenses for supplies, honorarium for the instructors as well as administrative expenses. Cancellation fees will be applied in case of cancellation.

Even though we state grade levels, these are a guide and should not be considered a restriction. With one registration you can attend more than one program, however, you will be able to choose only one of the programs in the registration form. • Grades 5-6:Wednesdays from 6:30 p.m. to 7:30 p.m.
• Grades 7-8:Wednesdays from 6:30 p.m. to 7:30 p.m.
• Grades 9-10:Mondays from 6:30 p.m. to 8:30 p.m.
• Grades11-12:Tuesdays from 6:30 p.m. to 8:30 p.m.

Note:  No Classes for Christmas break and no Classes for exam week.

All the sessions will run in room 105B Middlesex College
